Mission Trip Checklist

4 Months Out

    Apply for passport:
    • Passports can take 4-6 weeks to process, but allow up to 10 weeks
    Check on Visa information:
    • Required forms
    • Immunization requirements
    • Receive immunizations
    • It is a good idea to have your immunizations listed on a WHO card to carry with you.

3 Months Out:

    Check on Insurance requirements:
    • Review your current policy to check for coverage overseas.
    • Are services administered in Haiti covered?
    • What type of paperwork will you need to file?
    • Is there an injury or illness evacuation plan?
    • Consider purchasing supplemental travel insurance to ensure coverage.

2 Months Out:

    Check on baggage requirements:
    • Some airlines charge a fee for additional or oversized baggage.
    • Some items are prohibited on planes. See a list from TSA
    • Purchase tickets
    • If traveling with a group, confirm arrangements.

2 Weeks Out

    Put all required documents together.
    • Tickets
    • Passports and Visa
    • WHO card listed with vaccinations
    • Insurance cards, main and supplemental

How to get a passport

Applicants must apply in person at any of the following places:

  • Local Circuit Court Clerk
  • Most local Post Offices

Collect all of the following items and take them to one of the places listed above.

  • Fill out the application at the US Department of State website and print it
  • Social Security Card
  • Proof of identity - Examples: Driver's License, Government ID, Military ID or Naturalization Certificate
  • Birth Certificate
      Born in US: You can obtain a certified copy for a small fee from the Recorders Office in the county you were born
      Born outside of US: Naturalization Certificate or Certificate of Citizenship.
  • Two identical standard passport photos taken within the past 6 months. These can be taken at: Walgreen's, Walmart, FedEx-Kinkos, local portrait studios, and some post offices
  • Fees: Adult passport and card: $120
      Adult passport book: $100
      Adult passcard: $45 (Passcard not valid for international air travel. Used only for travel to Canada, Mexico, Caribbean, and Bermuda)

Make copies of information pages with your photos and personal data.
Take a copy with you and keep it in a safe separate place. This will make the process of obtaining a new passport in the event your original is lost or stolen.
